Home care after a hospital stay in Lakewood, CO: the first weeks home
If the stay was serious, Lakewood has already done its part. CommonSpirit St. Anthony Hospital at 11600 W 2nd Pl carries Colorado's highest trauma designation, a State-designated Level I Trauma Center, and works as a Comprehensive Stroke Center, with OrthoColorado Hospital alongside it at 11650 West 2nd Place. The harder question is the one no wristband answers: the first weeks back in a house in the 80228 or the 80214, when the person is weaker than they were last Tuesday.
We are a family-owned, non-medical agency. No wound care, no medication administration; instead, watching, reminding, cooking, keeping the house running, getting the follow-up appointments kept, and telling the family when something looks wrong. Here is what the first weeks home actually require, and which numbers in Lakewood's senior system stand between a recovery and a readmission.
Know the hospital before you need the hospital
St. Anthony's emergency department runs around the clock at 720-321-0000, and the campus heads Flight For Life Colorado's air ambulance service. CommonSpirit also lists Emergency & Urgent Care Lakewood at 3280 S. Wadsworth Blvd, reached on the system's shared ambulatory line, 303-673-7775. Write both down during the admission, not after discharge: which door you use for the new confusion at 2 a.m. is not a decision you want to improvise at 2 a.m.
The Level I trauma and stroke designation is why so many of this city's recoveries begin in that building, and it is also why discharge pressure is real. An acute bed is a borrowed bed, and the day it must be returned arrives fast.
Three questions to ask before transport leaves
Discharge rewards whoever showed up with a page. Ask them, in order: who is this person actually coming home to, hour by hour? Which appointments does the first week hold, and who drives to each? And what warning signs send them back, and to which number? The hospital answers in fragments. Writing them into one page is the family's job, and the caregiver can hold that page afterward.
The Lakewood version has specifics. A follow-up near a Federal Center station is easy by car and complicated by walker; the W Line light rail has run through the city since 2013 with seven stations, but platforms and paratransit are different problems. If the ride must be door-through-door, the city's van is appointment-based and cannot be flagged down.
The ride calendar IS the discharge plan
Lakewood Rides takes requests at 303-987-4826 for residents 60 and over and people with disabilities of any age, weekdays 7 a.m. to 5 p.m. from the Clements Center, appointment-only in the order requests come in. Destinations reach north to 49th Avenue, south to Quincy Avenue, east to University Boulevard or York Street, and west into Golden. A surgeon's follow-up on the other side of those boundaries is not the city van's errand.
RTD Access-a-Ride covers people whose disability keeps them off fixed buses and light rail, for any purpose, within 3/4 mile of a fixed route, reserved one to seven days ahead, with no same-day service; its administrative office is 303-299-2960, and the routes near your address belong on rtd-denver.com's lookup tool. Book the first post-discharge week's rides before the elevator reaches the lobby, and the calendar holds instead of collapsing.
What arrives funded, and what you fund
Two calls cover most of the free layer: the DRCOG Aging & Disability Resource Center at 303-480-6700 for no-cost options counseling and case management for adults 60 and over and adults with disabilities, and Volunteers of America's Meals on Wheels eligibility line at 303-294-0111, six days of delivered meals for people 60 and over at no charge. Seniors Resource Center in Wheat Ridge adds in-home help (303-235-6976), free chore work after a discharge like deep cleaning (303-235-6952), adult day and respite programming (303-235-6961) and booked rides (303-235-6972).
The county's front desk is Jefferson County Human Services Aging & Adult Services at 900 Jefferson County Parkway, Suite 180, in Golden, phone 303-271-1388 (toll-free 888-256-0321), open Monday through Thursday 7:30 a.m. to 5:30 p.m. and closed Fridays. It holds Adult Protection, Aid to the Aged Blind & Disabled, the Case Management Agency, the county Home Care program and long-term care eligibility. A recovery week is when that conversation gets started, if it ever does.
Week one inside the house
A non-medical caregiver's first week reads like chores and acts like medicine: the pillbox filled once rather than argued over daily, the fridge stocked with the recovery food, the follow-up appointment confirmed twice, trash and mail handled, and a note written about what the person actually looked like that day, not what they said.
The watching is the highest-value item on that list. A caregiver who reports the new redness, the fall that went unmentioned, the evening medication still sealed, has done the work of a test nobody ran. We are not clinicians and we will not pretend otherwise, but during our hours we are professional eyes in the room, and we call it in, using the after-hours number your discharge paperwork hands out.
Weather is a discharge variable, plan it in
Snow falls unevenly across this city: the National Weather Service's Boulder office puts the metro's western and southwestern suburbs in a 65 to 70 inch band per season against roughly 47 near Denver International, and Lakewood straddles both bands from Green Mountain and Bear Creek east to the plains. The nine highways through town, among them I-70, US 6, US 40 on Colfax, US 285, Morrison Road, Sheridan, Wadsworth, Kipling and C-470 at the western edge, can make Thursday's therapy appointment a judgment call rather than a routine.
Summer has its own version: wildfire season typically runs May through September, and CDPHE's guidance for older adults asks for a closed room, the HVAC fan on continuous recirculate, a MERV 13 or better filter and a HEPA purifier. On an air-alert day the errands become King Soopers pickup at one of the five Lakewood stores, and the library at 10200 W. 20th Ave runs curbside daily (303-235-5275), so a recovery week does not have to stall on weather it could have seen coming.
When the recovery outlasts the plan
Some discharge recoveries end. Some quietly change the household. If, by week three, the person cannot do what they managed before the admission, or the family's own schedules have collapsed around covering the gaps, the topic stops being the hospital and becomes ongoing help. In this metro band, care runs $43 an hour with three hours to a visit; when the need never ends for the day, live-in care costs a flat $650 per day instead of an evening tally.
We also help families read what a long-term care insurance policy actually says, and we are a VA Community Care Network provider, which matters in a city the Census counts home to 8,412 veterans. Where a recovery week becomes a new steady state, say so during the free 20-minute consultation, and we will price the real thing, not the emergency version.
Put the plan on paper this week
The best discharge outcome we see in Lakewood is boring: one page, four phone numbers on it, rides booked a day to a week ahead before the patient is even home, the meal route started, the warning-signs list in the kitchen drawer. The worst is the same family six weeks later, describing the missed appointment and the fall nobody witnessed.
If someone you know has a hospital date already, call before it. The 20-minute consultation is free, the assessment that follows, at the house or on video, is free, and neither commits you to anything. (303) 757-1777 reaches a real person, and planning the first week home is the cheapest medicine in this whole article.
