Memory & Dementia Care
Compassionate, specialized care for individuals living with memory loss

Dementia home care provides specialized in-home support for individuals living with Alzheimer's disease, vascular dementia, Lewy body dementia, and other cognitive conditions. Professional dementia caregivers are trained in redirection techniques, behavioral management, fall prevention, and creating structured daily routines that reduce confusion and agitation. Colorado CareAssist's memory care program includes personalized care plans developed with families, consistent caregiver assignment to build trust and familiarity, medication reminders, nutritious meal preparation, and engagement activities designed to maintain cognitive function. The Alzheimer's Association reports that over six million Americans currently live with Alzheimer's disease, with Colorado ranking among the faster-growing states for dementia prevalence. In-home dementia care allows individuals to remain in familiar surroundings, which research consistently shows reduces anxiety and behavioral symptoms compared to institutional care settings.
Expert Memory Care at Home
Dementia home care provides specialized in-home support for individuals living with Alzheimer's disease and other forms of dementia, allowing them to remain safely in familiar surroundings. According to the Alzheimer's Association, more than six million Americans are living with Alzheimer's, and the majority receive care at home from family members or professional caregivers. Colorado CareAssist provides comprehensive memory care across nine Colorado counties, with every caregiver trained in dementia-specific techniques including person-centered communication, redirection strategies, and sundowning management. Our services cover all stages of dementia from early-stage companionship and cognitive engagement through late-stage personal care and safety supervision. We work closely with families to develop individualized care plans that adapt as the condition progresses, helping reduce caregiver burnout while maintaining dignity and comfort for your loved one. Services for All Stages of Dementia
Early Stage
In the early stage, your loved one may still live independently but shows signs of change — repeating questions, difficulty managing finances or medications, misplacing items, and withdrawing from social activities. Our caregivers provide light assistance with daily tasks, medication reminders, companionship, transportation to appointments, and cognitive stimulation activities like puzzles, music, and reminiscence exercises. This is also the time to make home safety modifications and plan for the future.
Middle Stage
The middle stage is typically the longest and most demanding. Your loved one may need help with bathing, dressing, and grooming, experience confusion about time and place, and show personality or behavior changes including agitation and suspicion. Sundowning — increased confusion and restlessness in the late afternoon and evening — often appears during this stage and is one of the hardest challenges for family caregivers. Our caregivers are trained in sundowning management, behavioral de-escalation, structured daily routines, meal preparation, housekeeping, and wandering prevention strategies.
Late Stage
In the late stage, the person requires assistance with nearly all daily activities — eating, bathing, mobility, and communication. Care focuses on comfort, dignity, and quality of life. Our caregivers provide comprehensive personal care, feeding assistance, gentle repositioning, comfort measures, and 24/7 care options. Many families choose to keep their loved one at home with professional support rather than moving to a facility, and our team makes that possible with compassionate, consistent care.
24-Hour Memory Care at Home
For families whose loved one has progressed to moderate or severe dementia, 24-hour supervision becomes essential. Wandering, nighttime confusion, and fall risk don't follow a schedule. Colorado CareAssist provides round-the-clock memory care in your loved one's home — with caregivers specifically trained in dementia support, not just general personal care.
Choose between overnight shifts (a caregiver who stays awake through the night) or live-in care (a caregiver who lives in the home with scheduled rest periods). Both ensure continuous supervision.
Many families start with evening and overnight coverage during peak sundowning hours, then expand to full 24-hour care as the condition progresses.
For 24-hour care, we build a small team of 2–3 caregivers who rotate shifts. Your loved one sees the same faces every day, which reduces confusion and builds trust.
Shift handoffs include detailed care notes so every caregiver knows exactly what happened during the previous shift — medications given, behaviors observed, meals eaten, and sleep patterns.

Understanding & Managing Sundowning
Sundowning affects up to 66 percent of people with Alzheimer's disease (Alzheimer's Association). It typically appears in the middle stage and is one of the most common reasons Colorado families reach out for professional help. Symptoms include increased agitation, confusion, pacing, wandering, and difficulty settling down — usually starting in the late afternoon.
Close curtains before sunset, turn on bright interior lights before dusk, and reduce background noise to minimize triggers
Serve the main meal at lunch, limit caffeine after noon, schedule calming afternoon activities, and keep naps short
Calm, reassuring tone. Redirect rather than correct. Walk with them rather than blocking. Gentle touch when words fail.
Many families schedule professional care during late afternoon and evening hours — even a few days per week — to sustain the home care arrangement
